We are looking for an experienced cardiac nurse who would like to develop specialist skills in cardiac rehabilitation. This is an exciting opportunity for someone who is enthusiastic about secondary prevention for a wide range of cardiac patients who we treat in our cardiac centre. You will join an established multidisciplinary team and be involved in all components of cardiac rehabilitation from inpatient education and risk factor stratification after an acute cardiac event, telephone support and triage of ongoing symptoms, and continue the patient's journey into their core phase which involves exercise and health promotion.
You will be well supported and in time manage your own caseload of patients.
You will need to be a degree educated registered nurse with a strong cardiac nursing background and with evidence of post registration studies in cardiac care. In addition you need to have a Teaching and addressing module and basic and advanced life support skills.
Main duties of the job
To assist in the ongoing development, management and implementation of a service which facilitates the rehabilitation of both medical and surgical cardiac patients to the optimum level of physical and psychological wellbeing.
To manage own caseload of patients autonomously and undertake specialist nursing procedures e.g. coronary heart disease risk factor assessment and act upon findings making appropriate referrals as necessary to the smoking cessation nurse specialist, physiotherapists, psychology services or consultant physician.
To work as a team member within the Cardiac Rehabilitation team delivering a high standard of specialist nursing to cardiac patients and their families across all phases of cardiac rehabilitation.
This will be achieved by the provision of education on lifestyle changes to patients, their families, and carers during Cardiac Rehabilitation early and core phases, and through the discharge talks.
