We have a vacancy for a Care Co-ordinator to join the Wantage Primary Care Network. The Network supports just over 34,000 patients and comprises two Practices, Church Street and Newbury Street who are based in the same modern purpose-built health centre on the edge of the town centre.
This role will involve working closely with GPs and other primary care professionals within the practice and the Primary Care Network to identify and manage a caseload of identified patients. As a Care Co-ordinator, you will ensure that the appropriate support is made available to these patients and their carers in addition to ensuring that their changing needs are addressed.
Whilst advertised as part-time up to 30 hours per week, it could be combined with our vacancy for an INT Co-ordinator to become full-time.
Main duties of the job
We are seeking an individual who is enthusiastic, compassionate, approachable, organised and must be able to work as part of a team. The successful candidate must demonstrate good communication skills in order to provide timely and accurate verbal and written communication to patients, carers and a wide variety of healthcare professionals. Problem-solving skills and successful time management are also essential in the undertaking of this role.
IT experience is essential (MS Office & Internet) and a knowledge of EMIS would be an advantage however full training will be given.
Previous experience of working in a people-centred environment is preferred and knowledge of the local area is desirable, along with an awareness of the challenges faced in the community.
The successful candidate must have or must be willing to complete Safeguarding Certification and training set out by the Personalised Care Institute.
About us
We are a two practice Primary Care Network with a combined population of circa 34k. We share the same premises at Wantage Health Centre which has recently undergone an extensive development programme to provide additional consulting rooms and modernisation of the reception and administration areas. There is ample free parking on site but also good access to public transport. Wantage is a growing market town at the foot of the Ridgeway mid Oxford, Newbury, Reading and Swindon.
Both practices are teaching practices with a commitment to mentoring and supporting their staff. We would support the successful candidate to undertake further training and development.
We have an ongoing staff welfare programme and encourage all members of the team to come together for morning briefings, coffee mid morning and lunch time walks.
Job responsibilities
The Care Co-ordinator will:
1. Utilise population health intelligence to proactively identify and work with a cohort of patients (for example the elderly, disabled or at risk) to deliver personalised care e.g. patients with learning disabilities.
2. Support patients to utilise decision aids in preparation for a shared decision-making conversation.
3. Holistically bring together all of a person's identified care and support needs and explore their options to meet these into a single personalised care and support plan, in line with best practice.
4. Help patients to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
5. Support people to take up training and employment, and to access appropriate benefits where eligible.
6. Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.
7. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.
8. Explore and assist people to access personal health budgets where appropriate.
9. Provide coordination and navigation for people and their carers across health and care services, working closely with the care co-ordinator (safeguarding), social prescribing link workers, clinical administrators, and other primary care professionals.
10. Where appropriate refer people back to other health professionals.
11. Support the coordination and delivery of MDTs.
12. Work collaboratively with the general practice team to meet the needs of patients supporting the delivery of policy and procedures whilst managing own workload to deliver the practice priorities.
13. Raise awareness within the Network of shared decision-making and decision support tools.
14. Raise awareness of how to identify patients who may benefit from shared decision-making.
15. Ensure that all activities are recorded, monitored and evaluated.
16. Attend all meetings as required.
17. Work in accordance with policies, ensuring that all work and procedures are undertaken in accordance with issued guidance.
18. Attend any training courses and supervision sessions as required.
19. Promote the professional image and services offered by the practices.
20. Any other duties that may be required and agreed by the management team.
