Work with senior clinical leads within the Trust and local health and social care community to ensure delivery and development of the local palliative and end of life programme. Maintain and develop the Trusts end of life strategy and related policies in line with national guidelines and standards and identified local need. Ensure best practice is incorporated into end-of-life care plan and work with colleagues to ensure guidance and support for staff is up to date and readily available Ensure the maintenance of the governance and assurance framework for end of life care Provide trust wide clinical leadership in end-of-life care and lead the development of the Trusts response to National palliative and end of life care strategies Support the development of key strategies aligned to end of life care; dementia, long term conditions; frailty in line with national guidelines and local requirements Develop and review the palliative and end of life care and associated policies within the Trust and ensure awareness in all clinical services Ensure that a robust and efficient method of clinical activity recording, in line with National guidelines and recommendations, is in place Work with partner organisations to develop a forum to develop seamless care for patients Coordinate the reporting of performance for end-of-life care and ensure reports are timely and clear to support decision making
