Job summary An exciting opportunity has arisen within Cancer Services at York and Scarborough Teaching Hospitals NHS Foundation Trust. We are looking to recruit a Band 4 Associate Practitioner within the Breast Care Nursing Team, on a permanent basis for 18.5 hours per week (Thursday & Friday). This is a non-clinical role that involves both patient contact and administrative duties. As part of ongoing support all patients with breast cancer are offered a Macmillan Holistic Needs Assessment at key points after their diagnosis. A vital part of this role will be undertaking these assessments over the telephone in a virtual clinic. The other important aspect of the role will be administrative support to the breast care nursing team. To support the nurses with administrative tasks and planning to enable them to focus on their clinical duties. Further, the appointee must be equipped with excellent communication skills and the ability to work within a diverse multi-disciplinary group of clinicians, patients and allied healthcare providers. You must be self-motivated, enthusiastic, organised, computer literate and able to demonstrate excellent interpersonal skills, with a commitment to continued personal development. Main duties of the job Main duties of the role are: Provide a professional, proactive and sensitive service to cancer patients, their carers and relatives. Competently advise on and deliver health and/or social care to patients via the Holistic Needs Assessment. This will involve running telephone clinics, having supportive conversations and writing up care plans/actions for patient records. Be responsible for the coordination of the administrative function within the complex cancer pathways, chasing investigations, appointments, Multi-Disciplinary Team discussion and creating treatment documents in a timely manner, to ensure waiting time targets are not breached and patient's needs are met. Will act as a main point of contact for patients, carers and wider healthcare team to provide high quality patient care through proactive identification of needs, which requires a level of knowledge utilising approved tools and procedures. Collaborate and communicate with the wider healthcare team, acting as a resource to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of healthcare delivery. Will be expected to support the running of the service and work flexibly to the needs of the service, which can often be demanding and unexpected.
