Wootton Vale and Shortstown Surgery are seeking an enthusiastic clinical pharmacist to work on a full or part-time basis to join our teams working across both sites. As an experienced clinical pharmacist, you will provide the following; Carry out structured medication reviews, long term condition clinics, ensuring medicines optimisation and offering medicine information services to patients and clinicians. Work collaboratively with our healthcare team that includes pharmacists, GPs, specialist nurses as well as AHPs. As sess patients, devising, implementing, and agreeing management plans, and following approved protocols as appropriate. M ake onward referrals to other agencies and members of multidisciplinary teams. Training on local services will be provided. Other duties will include; Clinical medicines management. Provide face to face/remote structured medication reviews. Management of long-term conditions – asthma/COPD, CVD, diabetes, dementia/frailty. Medicines optimisation, review of monitoring needs and blood interpretation. Management of medicines on transfer of care and systems for safer prescribing. Manage prescription requests including authorisations and reauthorisations. Medicines safety and quality improvement, incorporating the use of national tools. Ensure both the public health and social care needs of patients within the GP practices. Regular audits to improve and influence prescribing across the practice. Support and supervise other staff in undergoing training. Please see the attached job description for more details. About the Candidate Essential: Master’s degree MPharm. Registered Pharmacist MRPharm. Postgraduate Certificate in Clinical Pharmacy or Pharmacy Practice (or equivalent). Mandatory CPD to maintain fitness to practice. Ability to work autonomously in managing own blood results and prescriptions. Minimum 2 years' experience of general practice and/or secondary care. Demonstrates an in-depth knowledge of medical conditions, treatments, and pharmacology. Experience of supporting doctors in clinical decision making. Ability to work autonomously within the scope of supervised practice. Detailed knowledge of maintaining confidentiality and implementing data protection legislation requirements including information governance. Desirable: Basic Life Support. Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice. Member of the RPS faculty. Independent Prescribing qualification.
