Adult Physical Health services are divided into 4 categories: Anticipatory, Planned and Unplanned Care, and Specialist services. All services aim to improve outcomes for people and maximize access to services through integrated working.
Working alongside the District Nursing Service with an Anticipatory Care focus, the aim is to reduce pressure on planned and unplanned care services. Supporting, teaching, and providing advice and support following best practice guidance is key to the role. The goal is to optimize patient care and ensure that patient independence is preserved and maintained wherever possible.
The Community Teams aim to maintain people within their own homes/communities and facilitate earlier discharge from hospital through a more coordinated approach to care.
The Community Teams work closely with colleagues in General Practice and Third Sector organizations. Central to the philosophy and approach is placing the individual at the center and proactively maximizing opportunities for choice, personalization, and self-care.
Main duties of the job
The post holder will work within standard operating procedures/pathways to manage their workload independently, with reference to a supervisor in the clinical area. They will support the assessment of patients and implementation of care programs, ensuring they practice within their sphere of competence and knowledge.

Person Specification
Experience
* Experience of organizing and prioritizing own workload
* Experience of working in health or social care settings without direct supervision
* Experience in a relevant clinical area (e.g., older people, pharmacy, community)
Qualifications
* Care qualification to NVQ 3 or equivalent experience
Skills
* Able to use judgment to prioritize tasks
* Good communication and interpersonal skills
* IT skills including word processing and emails
* Evidence of clinical skill development
