This is an exciting opportunity to join Angus Health and Social Care Partnerships Adults with Physical Disabilities Service.
You will be part of a team of social workers and care coordinators. Your duties will include providing a comprehensive person-centred assessment to people with physical disabilities, developing packages of care to help meet their identified outcomes and promote their independence, to enable them to achieve their full potential.
You will also conduct carer assessments and complete and maintain appropriate records. You will manage a caseload, carrying continuous responsibility for the assessment and evaluation of the provision of care and will participate in the continuing development of the service.

This statutory work allows Social Workers to work in more depth with supported people, build meaningful relationships, and support them to make safer choices with positive results.
This is a challenging, exciting, and rewarding role that offers the right candidates the opportunity to work within a supportive team environment, on the front line of social work, achieving the best outcomes for people with physical disabilities within our community.
