The post holder will support the Federations Service Lead and PCN Manager with the continued implementation of the nationally recommended Personalised Cancer Care Programme (Holistic Needs Assessment & Care Planning, Health and Well-being Event, Treatment Summary, Self-Management including risk stratified remote monitoring and surveillance). This will include day-to-day management and organisation of the Macmillan Cancer Care Support Coordinators working across the cancer teams within the PCNs, including line management duties and the operational development of the service.
Providing support to cancer specialists when implementing personalised cancer care, including assisting with holistic needs assessments and treatment summaries, organising health and well-being events, and providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.
Communicating effectively, daily and directly, face-to-face and via telephone with a range of individuals at various levels, e.g., patients, relatives, carers, other health and social care professionals, and community workers.
Providing general information and support about cancer and cancer services to enable navigation of health and social care and to help individuals make choices that are best for their cancer and life.
