Job Overview
We are seeking a compassionate and dedicated Support Worker to join our team. Duties
1. Provide assistance with daily living activities, ensuring the dignity and comfort of clients.
2. Oversee medication as prescribed and monitor clients for any adverse reactions or changes in condition.
3. Assist with case management tasks, including developing care plans tailored to individual needs.
4. Maintain accurate records of patient care, medication monitoring, and any changes in health status.
5. Communicate effectively with clients, families, and healthcare professionals to ensure coordinated care.
6. Accompany clients to appointments or social activities as needed, ensuring their safety during transport.
Experience
1. Previous experience in a supported living environment is highly desirable.
2. Familiarity with first aid procedures and emergency response protocols is advantageous.
3. Understanding of medication processes and patient care standards is essential.
4. A background in relevant healthcare qualifications will be considered an asset.
5. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
