Job overview
To work as a member of the Home Treatment and Rapid Response services, delivering support to patients and their families using evidence-based, profession-specific, and general mental health knowledge. You will provide client-centred, recovery-oriented principles and practice for individuals, families, and carers referred to the Service. Additionally, you will undertake assessments 24/7 received from the Urgent Mental Health Helpline as well as gatekeeping assessments. This role aims to provide an alternative to inpatient admission by treating patients in their home environment during a crisis with significant distress.
Service covers Dover/Deal, Shepway, and Ashford.
Main duties of the job
* Contribute to the assessment of clients referred to Home Treatment and Rapid Response.
* Provide support to clients and their carers regarding personal care, daily living, and social activities, helping maintain their social network through various home treatment interventions.
* Carry out specified interventions as per crisis plans and update accordingly.
* Monitor the progress of clients against crisis plans and communicate findings to senior staff for necessary reviews.
* Report any changes in client presentation and safety to Senior Home Treatment and Rapid Response staff.
* Work flexibly to respond to crisis situations and adjust priorities as required by the Shift Co-Ordinator.
* Engage clients and carers with crisis plans, negotiating available options while promoting crisis prevention, recovery, and resilience.
* Support carers to enable clients to remain at home, and assist clients with medication administration as per crisis plans when necessary.
* Promote the work of Home Treatment and Rapid Response to clients, carers, and other mental health professionals.
