Job summary Are you an experience Practice Nurse looking for career progression? We are seeking to recruit a Long Term Conditions Nurse for our practice. The successful candidate will take a key role as part of our skilled and dedicated Practice Nurse and HCA team. Main duties of the job The successful candidate would work to ensure a high level of care is delivered to all patients, particularly in areas such as long-term conditions. The practice is very supportive of any development opportunities, but is looking for a clinician that can really hit the ground running The role will include a mixture of face-to-face, telephone and home visit appointments to meet the needs of our patients. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required. The post holder will ensure that an equitable service is delivered to all patients including housebound patients. We are seeking an enthusiastic team player with good interpersonal skills. A knowledge of SystmOne would be an advantage. About us Our Practice Team consists of 5 GP Partners, Advanced Nurse Practitioners, Practice Nurses & HCAs, We have a very supportive and enthusiastic management team, excellent administrative and a reception team. We use the SystmOne clinical System and are a well-established Training Practice for GP Registrars and Medical Students. We are a recognised teaching practice for medical students from the University of Nottingham We are part of Ashfield North PCN and work collaboratively across our Network area. Our Primary Care Network team consists of Pharmacists, Physiotherapists, Mental Health Practitioners and a Pharmacy Technician, plus access to social prescribing team.
