Our team support a variety of social care duties and responsibilities in accordance with the Care Act 2014, and other relevant legislation. The team supports people living in care homes and their unpaid carers with responsibility for undertaking social care assessments and reviews, continuing healthcare applications, safeguarding enquires, mental capacity act assessments, and Court of Protection work. Our focus is on supporting people to achieve their individual outcomes to promote their wellbeing and working to ensure the provision of high-quality care. The role is varied on a day-to-day basis and allows for a high level of multi-agency working, creativity and autonomy.
