Job summary An exciting opportunity has arisen within All-Age Continuing Care for a Funded Nursing Care Practitioner to join our team. We are looking for highly motivated, compassionate, and skilled RN's who can work autonomously and collaboratively in a busy role to support the team, individuals, and their representatives in the assessment and review process of NHS Continuing Healthcare (CHC) and NHS-Funded Nursing Care (FNC). You will be responsible for line management and support of the Assistant Practitioners, whilst ensuring that comprehensive, person-centred assessments are undertaken so CHC and FNC eligibility can be determined.
