We are looking for an experienced minor illness/ emergency care practitioner or paramedic to join us who are enthusiastic, hardworking, dedicated and have a passion for delivering the best care to all our patients.
The ability to cover other aspects of General Practice will enhance your role - we are very aware that continual management of same day patients can lead to burn out of some staff. This might be Chronic Disease Management or Sexual or Women's Health - we are open to suggestions of how to keep you interested and motivated.
Main duties of the job
The successful candidate will work in our same day clinic as part of a busy team, providing care to a range of patients within your scope of practice. This will encompass the skills of assessment, examination, diagnosis, and treatment within an agreed scope of practice, working to current policies and protocols to ensure safe, appropriate, and legal practice are always followed. Paediatric nursing could be an advantage.
The ideal candidate will have qualifications and registration to support their clinical practice and will share our passion for patient care delivered in a local setting.
Excellent IT and communications skills are also a key requirement of the role.
Non-Medical Prescribing qualification desirable but not essential - a willingness to train is.
You need to be a team player who is able to work autonomously therefore experience and or qualifications in patient examination and clinical reasoning is essential. Mentorship, further training and development is available for the right candidate.
Depending on your experience we may ask you to undertake home visits to the elderly or housebound presenting with symptoms of minor illness.

Job responsibilities
Job summary:
* To provide care for the patients from presentation to discharge ina primary care setting, working with the professional boundaries as setout by the NMC, to demonstrate safe, effective, and appropriate care tomeet the needs of patients.
* To work effectively and supportively within the practice team,maintaining good levels of communication with both patients andcolleagues.
* Responsible for attending to patients who wish to be seen by ahealth care professional; diagnosing and treating patients presenting withminor illness; seeing and advising people in respect of their continuing medicaland nursing needs.
* Telephone triage and assessment is a large part of this role.
* The post-holder must hold a nursing or paramedic qualification andbe registered with their appropriate governing body.
To manage commonly occurring minor illnesses/injuries and to refer on toother health care professionals as appropriate.
* To assess, diagnose, plan, implement andevaluate treatment for patients presenting with undifferentiated diagnoses,making any necessary referrals to other members of the primary health careteam
* Undertake clinical examination as appropriate,ordering necessary tests as required to aid in diagnosis
* Interpret results of tests ordered and actaccordingly. The clinician should not order tests they are unable tointerpret
* Telephone triage
* Non-Medical Prescriber - prescribe and reviewmedication, appropriate to patient needs and in accordance with evidence-basedpractice, within your scope of practice
* Work with the patient to develop on-goingmanagement plans for health with an emphasis on self-care and prevention
* Ensure accurate documentation and recordkeeping, in accordance with NMC guidance
* Ensure clinical practice is safe and effectiveand remains within boundaries of competence and to acknowledge limitations
* Advise patients on general health care andminor ailments with referral to other members of the primary and secondaryhealth care team as necessary
* Collectionof pathological specimens including blood and swabs for microscopyrequested by other members of the clinical team
Administration and professionalresponsibilities
* Participate inthe administrative and professional responsibilities of the practice team
* Ensure accurateand legible notes of all consultations and treatments are recorded in thepatients notes
* Ensure theclinical computer system is kept up to date, with accurate detailsrecorded
* Ensureappropriate items of service claims are made accurately, reporting anyproblems to the Nurse Lead
* Ensure accuratecompletion of all necessary documentation associated with patient healthcare and registration with the practice
* Ensurecollection and maintenance of statistical information required for regularand ad hoc reports and audit
* Attend andparticipate in practice meetings as required

Training andpersonal development
* Trainingrequirements will be monitored by yearly appraisal and will be inaccordance with practice requirements. Personal development will beencouraged and supported by the practice. It is the individualsresponsibility to remain up to date with recent developments. The practiceactively encourages ongoing and appropriate personal development.
* Participate inthe education and training of students of all disciplines and theintroduction of all members of the practice staff where appropriate
* If it isnecessary to expand the role to include additional responsibilities, fulltraining will be given.
* Maintain own professional development andproduction of profession development portfolio, including a professionaldevelopment plan.
* Attend / undertake annual updates in mandatorytraining area
* As well as thenursing team, there is a need to work closely with reception and office toensure the smooth running of the practice, reporting any problemsencountered to the relevant person and ensuring everyone is aware of thedifferent roles within the nursing team.
* There is alsothe need to establish and maintain good liaison with other surgeries andagencies, including secondary care
Meetings
* It will benecessary to attend and contribute to various practice meetings asrequested. The only reason for not attending will be annual, study or sickleave.
Manage, supervise, co-ordinate and guide other members of the practiceteam
* Help to developand maintain practice policies and protocols for nursing activity
* Ensure the NurseLead is aware of any matters arising or problems / potential problems
* Ensure allsurgery-produced nursing leaflets are kept up to date

Person Specification
Qualifications
* Current Registration RGN or paramedic looking to come into Primary care.
* Degree module in Pathophysiology and Diagnostic Reasoning or a commitment to undertake
* Physical Assessment and Clinical Reasoning or a commitment to undertake
* Experience and evidence of learning equivalent to the above
* Independent Nurse Prescriber (Non-Medical Prescriber) or a commitment to undertake the qualification
* Minimum 2 years post registration experience
* Previous experience in Primary Care, ED MIU or WIC setting including care of minor illness patient
* Ability to work autonomously using own judgement, common sense and local knowledge to respond to the needs of patients.
* Maintenance of Confidentiality related to both patient and Practice business
* Able to work under pressure and emergency situations when the need arises
