Newtown Medical Practice is looking for a Practice Nurse to join their multidisciplinary team. The position requires an autonomous practitioner within their professional scope of practice providing care for patients via face to face consultations, telephone consultations, triage.
Applicants should be NMC Registered with Independent Prescribing. They should have a recognised Practice Nurse qualification working at degree level.
Main duties of the job
A candidate should have experience in management of minor illness, minor ailments and injuries and chronic disease management. They should be able to participate in patient telephone triage to manage patient demand and advise patients in the safest and most appropriate way relevant to their problems. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

We are a friendly forward thinking practice and have an experienced team of staff providing a high level of service to 13'600 patients.
The premises are modern, purpose built, well-equipped with a supportive working environment. The practice uses the emis clinical system and docman 10.
There are currently 4 full-time GP partners, 5 practice nurses including 3 independent prescribing nurses and 2 HCAs.

In order to work at this level NMC requirements for advanced practice must be met.
Job responsibilities
The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. As a practitioner the Nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.
Job Responsibilities:
Providing assessment, screening and treatment services and health education advice
Clinical practice
Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being ,within agreed protocols with referral to General Practitioners as necessary.
Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition in participation with General Practitioners or independently to agreed protocols
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
Work with patients in order to support adherence to prescribed treatments
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Organise and co-ordinate the provision of nursing services for the Practice to include the delegation of duties to other Practice nursing, Healthcare staff as appropriate
Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
Support and manage health needs of women presenting for family planning and cervical cytology consultations
Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
Implement and participate in vaccination and immunisation programmes for both adults and children
Advise, support and administer vaccinations where appropriate for patients travelling abroad
Promote and deliver evidence-based care for patients presenting with aural conditions
Meet the needs of patients presenting for opportunistic wound care
Provide a Minor Injuries service to Patients. Ascertain the nature of injury within 15 minutes of arrival and treat according, liaising with the duty doctor, as appropriate.
Assist in the management and follow up of patients in respect of Enhanced Services for Learning Disabilities, Diabetes, Heart Failure, Zoladex and any other appropriate Enhanced Service undertaken by the Practice appropriate to the Nursing Role
Seek and provide information to GPs and patients in respect of results
Undertake telephone triage, assessing and signposting the patient to the correct clinician
In principle, the Practice Nurse will perform all clinical tasks that she is capable of, and qualified for, which she is able to deal with instead of a doctor.
