Job Overview
The Reablement team/s will play a crucial role in providing high-quality support to individuals requiring assistance in their daily activities. This position involves working collaboratively with clients to promote independence and enhance their quality of life. The ideal candidate will possess strong communication skills, be empathetic, and have the ability to work effectively within a team environment.
Duties
Provide personal care and support to clients in their homes, ensuring their dignity and respect are maintained at all times.
Assist clients with daily living activities such as bathing, dressing, meal preparation, and medication management.
Work closely with other team members and management to develop and implement personalised care plans that meet the individual needs of each client.
Maintain accurate records of client progress and any changes in their condition, ensuring all documentation is completed in a timely manner.
Communicate effectively with clients, families, and healthcare professionals to ensure continuity of care.
Uphold health and safety standards within the home environment while promoting a safe space for clients.
Engage in training and development opportunities to enhance skills and knowledge relevant to the role.
Experience
Previous experience in a care or support role is preferred but not essential; training will be provided.
Experience in report writing and the ability to analyse information accurately will be beneficial for maintaining client records.
Working Pattern
We require alternative weekends
Shifts are 7am - 2pm and 4pm - 10pm
Full time staff are required to work 3 evenings a week
Part time staff are required to work 2 evenings a week
Qualifications
Excellent communication skills, including the ability to effectively interact with clients and colleagues
NVQ qualification (desired but not essential)
Experience in implementing care plans (desired but not essential)
Care certificate (desired but not essential)
