We have an exciting new opportunity for a motivated and enthusiastic Team Leader to join our memory service and lead the ongoing development and expertise of our care home team who provide assessment to diagnosis and beyond for service users with complex organic needs in the community who are residing in a 24 hour care environment. You will be responsible for the development and implementation of the day-to-day clinical leadership of the team, with the opportunity to develop creative solutions in assuring the delivery of excellent care to service users, their loved ones, care staff within care homes. The role is varied and will provide opportunity to develop and utilize skills within the team of direct organic needs interventions, leadership and education into 24-hour care settings, role modelling and the use of non-pharmacological interventions and pharmacological management with support from prescribers and medical staff. The team provide in-reach to all care homes within the locality,
enhanced care units and Navigo short and longer stay complex care units, providing an excellent blend of short and longer episodes of care. This is a challenging but rewarding role and we therefore seek a committed leader who will maintain and continuously develop the quality and clinical excellence in the service we provide., Offer clinical leadership and maintain, monitor and facilitate the delivery of a high-quality Mental Health Service
Provide strong professional/managerial and clinical leadership to the multi-disciplinary team
Provide strong leadership in respect of developing excellence in practice
Ensure National Standards and local guidelines, policies and care pathways relating to care and treatment are effectively delivered
Promote and ensure service users and carers are fully involved in the services they receive
Work across N E Lincolnshire providing mental health advice and leadership based on an agreed operational policy
Assist in developing and evaluation of models of care to enable effective communication
Participate in multi-disciplinary team meetings
A Registered Nurse/Occupational Therapist/Social Worker/AHP qualification
Degree or equivalent
1 year experience of leadership at Band 6
