Job summary Frimley ICB Complex Care Team is seeking to recruit a highly motivated, dedicated, skilled and experienced registered nurse to join our team, specializing in the co-ordination of care of patients with delirium, non-weight bearing conditions, and long term medical/nursing needs that cannot be provided by existing commissioned services. The ideal candidate will have experience in managing complex patient needs, providing high-quality care, and working collaboratively with all health and social care partners. You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication skills and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. You will be working in partnership with our local authorities, mental health, and community health providers so experience of multi-disciplinary working is essential. Main duties of the job Support with all functions of the Complex Care Team and duties outlined in the Standard Operating Procedure to expedite referrals in a timely manner to facilitate discharge and support with hospital flow. As a registered nurse to have specialist expertise regarding discharge and health and social care pathways playing a vital role within the hospital flow and being a point of escalation. Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes, other providers etc. Knowledge and experience of brokering placements to ensure appropriate levels of care and support to meet the patient's needs. Manage a caseload of complex cases. As an autonomous practitioner undertake all aspects of clinical duties according to the needs of the service.
