An exciting opportunity has arisen to be part of an innovative and dynamic multi-disciplinary team. Gateshead NHS Foundation Trust's Frailty Virtual Ward opened in September 2023. Since then the team has been preventing avoidable hospital admissions and facilitating safe effective discharges for those patients living with mild to very severe frailty. The team is supported by a Consultant Geriatrician as well as Community Frailty Nurse Practitioners. The post holder will have the opportunity to work with medical and nursing staff as well as allied health professionals, administrative staff and adult social care.
This post will involve working Monday to Friday, on a part-time basis between the hours of 8am and 6pm. The post holder will need to be flexible to meet the needs of the service. This is a community position and the post holder will need to hold a full UK driving licence and have a car available to use for work.
Main duties of the job
This post is for a band 5 staff nurse in a multi-professional team within Gateshead.
To work effectively within a multi-disciplinary/agency team attending to patients.
There will be planned and unplanned activity to provide hands-on care and holistic assessment.
This post is designed to support Frailty Virtual Ward Practitioners and the clinical operations manager to fulfill their objectives, ensuring the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients' health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty, palliative and EOL care needs.
This post will contribute to the safe delivery of a Frailty Virtual Ward for the patients of Gateshead, and will support the Senior Team in the safe and effective delivery of the service.
