Seeking a Family Partner who is a parent/caregiver raising or has raised a child with mental health concerns and is familiar with the associated challenges, OR a Bachelor’s degree with relevant personal/professional experience (self or family member) with childhood mental health issues.
Job Description:
The Family Partner will work as a team with a Clinician to provide child and family-driven mobile outreach services as a means of crisis intervention, hospital diversion, and skill building to support healthy family functioning and goal setting. The short-term mobile response team will work with children and families for a minimum of an initial meeting and up to 8 weeks as driven by the family.
In this role, you will:
Provide an array of formal and informal services and supports to families raising a child who is experiencing social, emotional, developmental, and/or behavioral challenges in their home, school, and/or community. Activities may include:
* Engaging the parent/caregiver in activities designed to address one or more goals on the waiver participant's service plan.
* Assisting the parent/caregiver in meeting the needs of the youth through educating, supporting, coaching, modeling, and guiding.
* Teaching the parent/caregiver how to network/link to community resources and treatment providers.
* Teaching the parent/caregiver how to advocate for services and resources to meet the youth’s needs.
* Guiding and supporting linkage to individual, peer/parent support, and self-help groups for the parent/caregiver.
The Family Partner will have first-hand knowledge of the services and supports available in the community and will offer resources including, but not limited to:
* Information on community resources.
* Assisting families in connecting to community resources and natural supports.
* Strengthening and supporting the caregiving efforts of families with emphasis on emotional, physical health, parenting, and family interaction.
* Empowering families to make informed choices regarding the nature of supports for themselves and their child.
* Developing a family’s capacity to actively participate in all decisions about services and supports for themselves and their child.
* Enhancing and improving the overall health and well-being of their child and family.
* Promoting effective collaboration and communication with the family and their provider team.
* Strengthening and developing a family’s skills and feeling of self-efficacy.
* Advocating with the family to access supports, services, and activities.
* Supporting families in caring for and strengthening their children’s mental and physical health, development, and well-being.
Qualifications:
The ideal candidate will:
* Be able to complete Mandated Reporting self-directed online training.
* Have experience working with children (preference given to those with experience working with children with special needs).
* Be a parent or caregiver of a child with a history of emotional and/or behavioral problems.
* Be able to complete the Individualized Care Model.
* Be able to complete and obtain their PEP Certification.
* Complete safety in the home community training.
* Complete OMH online Foundations training.
