Job summary
We currently have an exciting opportunity for an Onsite IP Clinical Pharmacist, to work full time for Maryport Health Services within Cumbria, this opportunity is available for highly motivated and focused Clinical Pharmacist, who will be responsible for the smooth running of day-to-day activities within general practice, while playing a vital role in providing pharmaceutical care and advice to patients focusing on best practice and adhering to safety guidelines.
Full time hours can be worked within core opening hours of the GP
Main duties of the job
The Role
As a onsite Clinical Pharmacist you will confidently act within professional boundaries, supporting and working alongside your general practice team as well as pharmacists and Pharmacy Technicians within TPN.
You will help resolve medication-related issues, help utilise skill mix, improve patient outcomes and support the improvement of access to healthcare.

Main duties of the job
10. As a clinical pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's.
11. Resolve medication-related queries
12. Polypharmacy medication reviews and SMRs
13. Telephone general medication reviews
14. Improve patient outcomes
15. Ensure better access to healthcare
16. Support practice staff with prescription and medication queries
17. Medicine reconciliation and review post-discharge
18. Conduct medical related audits and work on service improvement with the practice
19. Provide expert advice on medications to other clinicians and patients
20. Deliver clinical leadership on medicines optimisation and quality improvement
21. Ensure appropriate and efficient use of medication
22. Educate patients on medication and disease prevention
23. An understanding of QOF, CQC and local incentive scheme workstream
24. Create and follow clinical pharmacy programme according to practice policy
25. Excellent communication and interpersonal skills
26. Conduct long term condition reviews

Qualifications
Essential
27. A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
28. Independent prescribing qualification
Desirable
29. Desirable to already be on CPPE Primary Care pathway or will be willing to partake, if you have extensive experience working as a Clinical Pharmacist an exemption route can be sourced.
