We are looking for an enthusiastic and motivated Band 7 Clinical or Band 8a Advanced Clinical Pharmacist (depending on experience) to be part of our pharmacy team covering a team members maternity leave. This post is a one year fixed term contract.
Our team provides clinical pharmacy and medicines supply services to Mental Health, Learning Disability and Community services across the Trust. Our pharmacists and pharmacy technicians are valued members of the multidisciplinary teams, and the service is currently in the process of implementing a strategy to expand our clinical services and deliver a high-quality medicines optimisation service across all trust services. We have also recently introduced electronic prescribing and medicines administration system across the Trust.
The postholder will be responsible for the delivery of day-to-day medicines optimisation and clinical pharmacy services to patients within our care in the Wakefield / Kirklees localities. If the successful candidate meets the requirements for the Advanced Clinical Pharmacist (8a) role, then they will have the additional responsibility of line management and overseeing a clinical pharmacy service to a designated area.

Main duties of the job
The post will be based across Fieldhead hospital in Wakefield and The Priestley Unit in Dewsbury (but the postholder may occasionally be asked to work at other Trust sites).
The post holder will be responsible for providing clinical pharmacy services to designated inpatient units/crisis team/community teams with involvement in the delivery of patient care, contributing to MDT meetings and delivering education to patients and professionals.
The successful candidate will also support the departmental and Trustwide governance agenda, providing clinical input into audit and service development projects. They will support the day-to-day operation of the service, providing training and staff supervision as necessary.

Job responsibilities
1. Provision of Clinical Pharmacy service to allocated teams.
2. Provision of high quality patient-focused pharmaceutical care.
3. Provision of professional responsibilities of a pharmacist supervising the dispensing of prescriptions at Fieldhead Pharmacy whilst fulfilling the dispensary commitment.
4. Participation in the Saturday, Bank Holiday and On-call Rotas.
5. In conjunction with the pharmacy team, develop and implement safe and effective policies and procedures in line with New Ways of Working.
6. To provide management to allocated staff including appraisal, development and clinical supervision.
7. To be involved in the development of new services as allocated by pharmacy management team.
8. Develop and participate in Trust-wide medicines management audits and attend allocated governance groups to represent pharmacy.
9. To be responsible for the clinical pharmacy and prescription monitoring at allocated clinical teams. Be a specialist medication referral point for these teams.
10. Medicines reconciliation targets for inpatient area.
11. Ensure all service users have access to clinical pharmacy services.
12. Screening for all service users admitted to intensive home-based treatment services.
13. Develop and contribute to patient-focused services to allocated teams, including service user groups.
14. Provide appropriate and timely verbal and written information on all aspects of medication usage to service users, carers and healthcare professionals within SWYPFT.
15. Participate in in-house training for pharmacy staff and healthcare professionals, both attending and delivering training.
16. Participate in the training of pre-registration pharmacy graduates and mental health pharmacy diploma students.
17. Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units.
18. Participate in research and development as required for the development of the clinical pharmacy services.
19. To cover for other senior pharmacists in their absence.
20. If required due to sickness or other absence to work in other localities of the organisation.
21. To carry out CPD in line with the GPhC requirements.
22. To maintain and demonstrate a good clinical pharmacist knowledge.
23. To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from other organisations when working in partnership.
24. To participate in the Trust's Appraisal system.
