Position: Physical Therapist
HSS Rehabilitation provided by Stamford Health is seeking a motivated Physical Therapist to join our Outpatient Rehab team. We are part of a groundbreaking collaboration between Stamford Health and Hospital for Special Surgery (HSS). Our clinics are located in Stamford, Greenwich, New Canaan, Norwalk, Wilton, and Westport, with opportunities throughout the region.
Qualified candidates should have experience in non-operative and post-operative patient management. Strong communication skills, leadership abilities, commitment to lifelong learning, and flexibility to work collaboratively are essential. Connecticut licensure is required; advanced certifications are preferred. New graduates are encouraged to apply, and strong mentorship is available.

Responsibilities
1. Evaluation: Obtain pertinent medical and social history. Perform thorough evaluations to measure physical and functional status. Set short and long-term goals and design appropriate treatment plans, considering discharge options from the initial evaluation.
2. Treatment: Implement treatment plans, modify as necessary, update goals, and re-evaluate discharge plans throughout treatment. Include patient and family education and communicate with team members.
3. Productivity: Meet departmental productivity requirements and goals.
4. Documentation: Ensure timely, objective, and complete clinical documentation, billing, and outcome measures.
5. Communication: Interact professionally with interdisciplinary staff, patients, and families, demonstrating respect and compassion.
6. Education: Engage in continuing education, provide education to staff and community, and stay updated with clinical research and best practices.
7. Supervision: Supervise assistants, aides, volunteers, and potentially students after a minimum of 2 years of experience.
Qualifications
* Doctorate of Physical Therapy from an accredited program (Bachelor’s degree accepted for those licensed before 2015)
* Valid Connecticut Physical Therapy license or eligibility
* BLS certification
* New graduates may be considered
