Job summary
We are pleased to be able tooffer this full-time Band 5 Physiotherapist role with the expectation of thesuccessful candidate being able to progress to Band 6 upon completion of theBand 6 Senior Physiotherapist competencies. This may take 6 months to 2 yearsdepending on your current experience and skills.
The Community Rehabilitationand Falls Service works with a diverse caseload of patients in need ofrehabilitation following an acute illness or injury, as well as those withdeteriorating conditions where the focus is maintaining function and independence.The service has a particular specialism in falls prevention and rehabilitation.
Working in this field ischallenging in its diversity, but offers an opportunity to use and build skillsin key areas such as Falls, Neuro, Frailty, and MSK. Supervision, support and training will beprovided by Senior Physiotherapists within the team, as well as SeniorOccupational Therapists, and our very experienced and capable AssistantPractitioners and an Administrator who will further support your development.
Main duties of the job
The role involves providingphysiotherapy assessment and intervention for rehabilitation, promotingindependence, and falls prevention. With a varied caseload, you will need toevidence at application and interview that you have at least undergraduateexperience in Neurology, MSK/Orthopaedics, and Health Care of the Elderly. Postgraduateexperience would be an advantage.
You will work alongsidecolleagues within a multidisciplinary team to deliver exercise programmes, gaitrehabilitation, provision of mobility aids, and falls prevention strategies. Asthe assessment and treatment takes place in patients own homes, you will needto be a car driver in order to carry out the role.
Experience of workingin the community and as part of a multi-disciplinary team will be advantageous,and may mean that you are able to progress to a Band 6 role more quickly thanif you do not have this experience at the point of application.

Job Purpose
Tocontribute to a high-quality service by carrying out assessment, goal setting,implementation and evaluation of treatment programmes, within patients ownhomes, of adult clients who may have complexand/or chronic presentations including musculoskeletal, sensory impairment,medical and mental health needs, with support and supervision from SeniorPhysiotherapists.
To support clients to improve or maintaintheir level of function and quality of life, and in particular to reduce therisk of falls, in partnership with the multi-disciplinaryteam (MDT). This will include liaising with and referring on to other healthservices, voluntary services and public sector agencies, and planning safe andeffective discharge from the service.
To participate actively in the servicestraining strategy, including in-service training, organisation-specific andmulti-agency training.
Dimensions
Responsible for Physiotherapy assessment andtreatment for a defined clinical caseload appropriate to the service area,working in conjunction with and under supervision of a Senior Physiotherapistand other senior clinical professionals.
To participate in rehabilitation treatment programmes with the MDT,including leading group rehabilitation classes as appropriate.
To contribute to the supervision and training of Assistant Practitionersand Rehabilitation Support Workers, and to support senior colleagues withdelivering student placements
Able to fulfil physical demands of role, including manual handlingapplications.
Will be required to travel city-wide for patient visits, meetings andtraining.
The role has no specific budgetaryresponsibility, however there will be a requirement to demonstratecost-consciousness within the role.
Key Responsibilities
Clinical and professional
1. Responsiblefor physiotherapy assessment and treatment for a defined clinical caseload,working in conjunction with and under supervision of a senior physiotherapistand other senior clinical professionals.
2. To beprofessionally and legally responsible and accountable for all aspects of yourown work including the management of patients in your care.
3. Toensure a high standard of clinical care for the patients under your management,and support other team members to do likewise, implementing appropriateteaching, supervision and delegation skills where appropriate.
4. To workholistically across role boundaries to Band 4 level to assess for, provide,demonstrate and fit adaptive equipment using problem solving, and analyticalskills.
5. Toorganise own clinical caseload effectively.
6. Workingwithin a legal framework, gain informed consent to treatment and have theability to identify, through assessment, patients who lack the capacity toconsent.
7. Todemonstrate a sound understanding of Clinical Governance and Risk Managementand apply to a work situation. To conduct and report on clinical riskassessments as and when appropriate.
8. Toundertake the comprehensive assessment of patients, including those with acomplex presentation with the support of senior physiotherapists. Formulateindividualised treatment plans with rehabilitation goals using clinical reasoningin consultation with senior therapists as required. This should be based onsound theoretical knowledge, utilising a wide range of treatment skills, andoptions to formulate a specialised programme of care.
9. Toadvocate on behalf of patients and carers, where appropriate, overcomingbarriers to communication such as cultural or language difficulties, physicalor mental health difficulties, using skills of empathy to facilitate or empowerthe patient.
10. To beinvolved in the development of good practice for physiotherapists withinNottingham CityCare Partnership CIC and Social care under the guidance ofsenior staff.
11. Tocomplete patient records in line with organisation, regulatory body and legalrequirements.
12. Todemonstrate a commitment to evidence-based practice and the use of outcomemeasures and make recommendations for change. To have a good understanding ofthe research process, and be willing to participate in local research and auditwhen appropriate.
