In the role of Associate Practice Development Facilitator, you will be responsible for planning, implementing, and evaluating programmes of palliative and end of life care education across a range of diverse care settings in the Borough, including the hospital, community and care homes.
You will participate in role modelling work with staff in their clinical settings, so enhanced communication skills are essential – together with being a team player and the ability to work autonomously. You will have the opportunity to participate in the practice development of clinical services through audit, research and education, and as some education is now delivered remotely using technology, you are required to be IT literate.
The postholder is required to hold a first level registered health or social care qualification (e.g. nurse, paramedic, social worker or other allied health professional). Evidence of teaching within a clinical setting, understanding the palliative and end of life care learning needs of staff across primary, secondary, and tertiary care and demonstrating post registration experience in palliative care/relevant field to the role is also required.
The main base of work will be at the hospice; however, there will be regular travel to support staff across many care settings, therefore a full UK driving licence and access to a car insured for business use is essential.
If you have not heard from us within two weeks after the closing date, this will mean you have been unsuccessful this time.
Successful appointments will be subject to Enhanced Disclosure and Barring Services Check (DBS) check.
