An exciting new opportunity is available for a Registered Nurse to join our newly restructured Planned Integrated Care Service.
Planned/Unplanned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post will be located in the North of the Barnet localities. The post holder will work closely with the team leads/ clinicians to develop the service and maintain close working relationships with both acute and community services.
You need to be compassionate person, able to show empathy for patients and their carers. Candidates must hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
The post holder will undertake community nursing duties involving delivery of nursing services and holistic assessments of patient’s health needs using a person-centred approach.
This includes providing quality and seamless nursing care to housebound patients and being proactive in co-ordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care institutions.
In addition, the post holder will be involved in facilitatingself-carefor patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.
