Young People Leaving Care Support Worker – Manchester
Position Overview: This is an outreach position that involves driving to meet young people for appointments. The office is located in Manchester.
Essential Requirements:
1. Full UK Driving License with Business Insurance is essential.
2. Experience supporting young people and their families.
3. High standard of general education.
4. Preferably: NVQ Level 3 Health & Social Care – Children and Young People (CCYP) or equivalent.
5. Willingness to undertake further training.
Job Purpose: To be responsible for the provision of support for vulnerable people (service users) for the company.
Main Duties & Responsibilities:
1. Assist the young person’s allocated Social Worker in service delivery.
2. Provide transport/escort to the young person as directed in line with care plans.
3. Conduct welcome and induction for young people.
4. Ongoing assessments of service users’ needs.
5. Provide ongoing support and monitoring of service users.
6. Assist in welfare and NHS issues.
7. Preparation of reports.
8. Arrange appointments with various partner agencies.
9. Maintain regular contact with service users.
10. Inform service users of support groups.
11. Assist in the move-on of service users.
12. Accurately record, create, maintain, and update electronic records of service users.
13. Prepare for and attend meetings as required or directed.
14. Provide mediation to service users as required and/or directed.
15. Effectively use IT packages to support the work of the company.
16. Work effectively and cooperatively across multi-disciplinary teams.
17. Observe and maintain strict confidentiality regarding service users, clients, and the company.
18. Proactively assist in the development of company policies and procedures.
19. Report any housing issues to the Services Department and assist in the resolution of defects.
20. Undertake on-call duties as directed.
21. Participate in office reception duties when required.
22. Work with service users on a 1:1 basis or support groups of 3 young people 24/7 where sleep-in shifts are required.
23. Show flexibility to cover all offices across the company as needed.
Working Hours: 9-5 Monday to Friday, with flexibility required to meet the needs of the young people.
Compensation: £14.50 per hour and 40p mileage.
This is an immediate start position on a long-term temporary contract, with the potential to become permanent after 12 weeks if preferred. Otherwise, it will be an ongoing temporary contract lasting at least 3 months.
