An exciting opportunity has arisen for a Community Mental Health Nurse position within the Mid-Home First Team based in Chelmsford. We are seeking a motivated and enthusiastic individual who is passionate about community mental health. The post holder will join an evolving team and will be jointly responsible for the delivery of high standard community care, forming part of a coordinated plan of home treatment to Service Users predominantly within their own homes.
Main duties of the job
The post holder carries responsibility for the assessment of care needs and their development, implementation, and evaluation of programmes of care for Adults and Older Adults with mental health problems that are referred to the Crisis Resolution Home Treatment Team. The post holder is responsible on a day-to-day basis to the Clinical Lead but ultimately accountable to the Matron for the overall service. In addition, they will be a member of a multi-disciplinary team and work in close cooperation with wards and Primary Health Care based professionals. They will also be responsible for the maintenance of standards of care within the Crisis Resolution Home Treatment service, ensuring that an integrated service is provided to their clients and that a good inter-disciplinary working environment is maintained.
Job responsibilities
1. To establish and maintain effective individual health and social care by:
1. Holistic assessment of client problem/needs.
2. Planning, implementation, and evaluation of interventions.
3. Clear, accurate, and concise documentation of client care.
1. To participate in the initial assessment of clients to determine appropriateness for the Crisis Resolution Home Treatment Service and to liaise with referring agencies to identify alternative agencies as appropriate to meet the clients' needs.
1. To participate in Team and Clinical Meetings, initiating and discussing new methods/models of care and in the introduction of research or trials within the area.
1. To be fully aware of the importance of effective risk management, the undertaking of clinical risk assessment, and the ongoing monitoring of the clients' mental state to evaluate risk either to themselves or towards others.
Person Specification
Education / Qualifications
* Professional Qualification (OT, Social Work, RMN)
* Educated to Degree level in Health.
* Evidence of personal and professional development in contemporary mental health practice.
* Allied Health & Mental Health Practice.
* Additional Professional Qualification/Experience.
Knowledge
* Ability to coordinate work to meet agreed targets and deadlines.
* Evidence of ability to prioritise workload and competing demands.
* Able to take on broader responsibilities to improve own and others' knowledge and skills.
* Evidence of the knowledge and skills in the use of information technology to support care and professional development.
* Good understanding of the Mental Capacity Act and its application in practice.
* Good understanding of person-centered care and Recovery Model.
* Good understanding of crisis resolution home treatment service provision.
* Good understanding of positive risk taking.
* Good understanding of national policy and the implications for mental health e.g. crisis care concordat, parity of esteem.
* Evidence of understanding the principle of governance and implications for professionals practice.
* Evidence of knowledge of professional nursing standards.
* IT Competency.
* Car Driver / able to travel to all required locations and achieve service activity.
