Job summary Wansford and Kings Cliffe Practice has a vacancy for an experienced Practice Nurse for 30 hours a week. To be worked Monday, Thursday and Friday 8am to 6pm ( with one hours unpaid lunch break). There may be the opportunity to develop into a nurse lead role should you have or develop the necessary experience. The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population across both Wansford Surgery and our branch surgery, Kings Cliffe. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels. a. To assist the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager b. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice c. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing d. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health e.
