Job summary The post holder will be an integral member of the Enfield Adult Community Physiotherapy and Adult Neurological Therapy teams. The post holder will be developed to take short term responsibility for the effective planning and delivery of care to patients in co-operation with other team members and in communication with other clinical team leaders. The post holder will be required to support clinicians in running gym based groups and education programmes and also to visit patients in their own homes to carry out prescribed treatment and assess for and provide appropriate O/T equipment or mobility aids directly or by prescription. The post holder will work alone and will be required to use a degree of initiative while working within the parameters of the team's policies and procedures. The post holder may be required to work at times in other services dependent on integration and service need. The post holder will be an integral member of the multi professional teams, which is in partnership with the voluntary sector in delivering and their carers. The service provides multidisciplinary therapy (OT and Physiotherapy) and support to help patients engage in a rehabilitation or long term management programme to regain or retain their independence. The post holder will provide health care to clients under the supervision of the Senior Therapists within the team. To be responsible for a designated caseload of patients. To work effectively as a member of a team in providing a programme of community based therapeutic activities, Communication To contribute to the effectiveness of team working through timely and appropriate communication between all work colleagues within Enfield Community Services and its partner organisations. To maintain accurate timely written records of all interventions and communications including letter writing, sketch plans and discharge reports. To use information technology for communication; voicemail, email and word processing and collecting data for performance indicators/ outcome measures on clinical information systems. Information Management To be responsible for accurate recording of required data on patient management systems eg RIO Effort, Mental and Environment To be able to deliver and install temporary adaptive equipment in clients own homes. To be able to operate manual handling equipment i.e hoist, rota stands, mobility aids, To be able to perform therapeutic manual handling techniques with patients. To be able to operate computer systems after a period of training. The able to produce internal reports of client's progress. To deal with a range of patients and carers who may be distressed.
