Key Tasks: To work with the Cluster Pharmacist and GPs to carry out Medication Reviews To work with GPs and practice staff to ensure that all repeat prescriptions produce at the practice are ready within 2 working days of request including My Health Online requests To work with the Prescribing Hub to ensure that all repeat prescriptions requests are issued if appropriate and ready for collection within 3 working days To deal with patient and staff queries via the phone line or front desk in a timely manner To carry out any Audit work deemed necessary by the practice around medication or clinic work. To make yourself competent in running drug searches To keep an audit trail of prescription bags to the doctors ensuring they are returned All new drugs are added to the patients medical records Liaise with community pharmacies, hospital and patients ensuring a good working relationship is maintained Daily collect all TOC reports and make changes to patients medications as necessary Liaise with Community Drug and Alcohol Team and Alternative Healthcare Primary Care Scheme doctor re issue of medication for patients on those schemes Ensure any monitoring tests are up to date before issue of prescription LHB to carry out all LHB requested work Attend all relevant training offered to support the prescribing role To provide admin support for visitors/supervisors/managers/ and clinical members of the surgery To familiarise yourself with the processes of remit of other members of your team to cover for holidays and sickness As an employee of the Practice, you are legally responsible for all records that you gather, create, or use as part of your work within the Practice (including Patient health, financial, personal and administrative (whether paper based or electronic. All such records considered as public records, other than patients personal notes, and you have a legal duty of confidence to service users, even when you have left the Practice. All employees of the practice are required to maintain the confidentiality of members of the public and members of staff. The post holder will be subject to an annual appraisal, which will incorporate a review of the above duties and the performance of the post holder.
