This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence-based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
