Lead Practitioner
Location: The roles will be based within South Tyneside Adult Recovery Service (STARS) and work within community venues across South Tyneside.
Must hold a valid UK Driving Licence and access to a Vehicle.
About the role
We are looking to recruit a highly motivated Lead Practitioner to work alongside South Tyneside Adult Recovery Service (STARS) and local partners to improve health outcomes for individuals in South Tyneside, particularly those who sit within inclusion health groups and may be socially excluded and/or experience multiple overlapping risk factors for poor health.
The most important things you’ll bring to this role are your positive attitude, enthusiasm, integrity and ability to use initiative as well as your belief in our ambition to reduce barriers and improve experiences ensuring individuals to have better access to health and care services.
You will provide line management support and supervision to a small team of Health Navigators and have responsibility for management of the project day to day, working closely with the Area manager to ensure that the project delivers agreed interventions to meet the needs of our stakeholders and the community.
