Patient Pathway Coordinator for Virtual Hospital Services
This is an exciting opportunity to be involved in the expansion of the RBFT's Virtual Hospital Services (VHS), which is at the forefront of the new specialty of 'Virtual Medicine'.
Virtual Hospital Services care for patients, who could benefit through admission alternatives or early supported discharge, with remote and hospital at home management, in a place they call home.
The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
Please note: This role is provided in two locations; within the Virtual Hub, based at the Royal Berkshire Hospital, and at the Virtual Spoke, located at the University of Reading's Whiteknights campus.
The successful candidate will be required to work at both locations as part of a rota.
We offer flexible working and shift patterns that cover 7 days a week. There is a requirement to work weekends.
Main duties of the job
1. The successful candidate will work directly alongside the Multi-Disciplinary Team (MDT), to safely and efficiently manage and care for acutely unwell patients remotely.
2. They will act as a point of contact to manage patient interactions and communications, and facilitate assessments, treatments and escalation to the appropriate member of the MDT.
3. You will be responsible for tracking patients across their journey and documenting referrals interactions and communication within the EPR.
4. Will be responsible for coordinating the distribution and delivery of pharmaceuticals, patient information and medical equipment via our logistic services.
5. Demonstrate high levels of compassion and be an ambassador for excellence in patient experience.
6. Undertake clinical audit, whilst managing patient level data through our EPR and database systems.
7. Support the Service Manager in the development and delivery of new pathways and practices.
