We are a highly successful, 8 partner practice, serving a population of over 15,000 patients in stunning Pembrokeshire. Main duties of the job
Clinical Responsibilities:
1. In accordance with the practice timetable, the post holder will undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, administration, and correspondence in a timely fashion.
2. Making professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other health care workers within the organisation.
3. Undertake on-call duty on a rota basis with other GPs.
4. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
5. Screening patients for disease risk factors and early signs of illness.
6. In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
7. Providing counselling and health education.
8. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
About us
We are a large practice of 15,600 patients covering a wide area of Central Pembrokeshire. We are based in the County town of Haverfordwest and serve both the urban and surrounding rural population. We have 8 GP Partners who are supported by a wide MDT of 1 part-time Salaried GP and 3 Advanced Nurse Practitioners. We also are supported by Physiotherapists and a Mental Health Counsellor within the Practice. The practice is a training practice supporting registrars in the local scheme. We are a friendly and supportive Practice which aims to deliver high quality and consistent care to our patients.

1. Awareness of and compliance with all relevant practice policies and guidelines, e.g., prescribing, confidentiality, data protection, health and safety.
2. A commitment to lifelong learning and audit to ensure evidence-based best practice.
3. Contributing to evaluation, audit and clinical standard setting within the organisation.
4. Contributing to the development of computer-based patient records.
5. Contributing to the summarising of patient records and Read Coding patient data.
6. Attending training and events organised by the practice or other agencies, where appropriate.
