This is an exciting opportunity to join Manchester’s expanding Adults Early Support Team; a city wide, multi-disciplinary team enabling the adults of Manchester to live their best life. The Adults Early Support Team has emerged from Manchester’s programme of practice led, transformational change, Better Outcomes Better Lives.
With a focus on maximising independence and strength based conversations at the first point of contact, AEST deliver short term interventions providing the right service at the right time in the right place.

About the Candidate

Aims and Objectives
* Work with adults to maximise independence meeting need in creative ways.
* Deliver a timely and efficient intervention and prevent needs from escalating.
* Manage pressures and demands on long term services.
* Network and build links with community assets, work collaboratively with partner agencies including the voluntary sector, primary and secondary health services, Children’s Services.
* Incorporate ‘Our Manchester Behaviours’ into everything that we do.
To meet Manchester’s Prevent, Reduce, Delay duties under the Care Act 2014.
Hybrid working: You will be expected to be office based when on duty.
