Mottingham, Downham & Chislehurst Primary Care Network (MDC PCN) are looking for an experienced self-motivated, forward thinking qualified pharmacist to join their pharmacy team.
Main duties of the job
1. Medicines optimisation within the PCN
2. Providing expert advice on clinical medicines
3. Undertaking structured clinical reviews with patients to manage complex polypharmacy needs
4. Working as part of a Multi-Disciplinary Team (MDT) in a patient facing role to clinically assess & treat patients
5. Provide professional advice & support on the safe prescribing, handling & administration of medicines. This includes support regarding development and implementation of medicines management processes & policies
6. Lead on the medicines aspects relating to the care of patients to optimise medicines, reduce medicine-related risks & improve adherence to taking medicines by carrying out medication assessments & reviews in identified patients
7. Resolve day-to-day medicines & prescription issues with healthcare providers
8. Help patients manage their conditions
9. Lead prescription services & deal with queries
10. Present to patients any changes to prescribing in line with NICE, best practice or in response to prescribing alerts
11. Rationalise repeat prescription lists to avoid waste/duplication
12. Provide reviews to those patients with long term conditions or complex health needs, including drug monitoring & risk vs. benefit
13. Contribute to the management of the prescribing budget through the promotion of safe, cost-effective rational prescribing
14. Contribute to minimising the harms associated with the use of medicines
Person Specification
Qualifications
* Registered with the General Pharmaceutical Council
* Postgraduate qualification in clinical/community pharmacy or equivalent additional experience
* Evidence of recent continuing learning/professional development
* Diploma in clinical/community pharmacy
Experience
* Experience of working in multi-disciplinary teams
* Experience of medication reviews
* Experience of working within Primary Care
* Experience of Emis GP Computer System
* Knowledge of the changing NHS environment and structures
* Familiarity with analysing/interpreting ePACT data
Skills & Abilities
* Excellent interpersonal skills
* Ability to work on own initiative, able to plan, prioritise, make decisions and meet deadlines.
