Driver Essential? Yes, to travel to and from family home.
Essential: Experience supporting a young person with Autism Spectrum Condition (ASC), experience in a senior, managerial or team leader role.
Start Date: ASAP
Days & Hours: 36 hours on average per week. Monday to Friday: Term-time 3.30pm to 9.00pm and School Holidays 10.00am to 6.00pm. 5 hours allocated for admin / team lead duties each week. Role can be offered on a part-time basis working 3 plus shifts a week, please get in touch with your availability.
About this client/child: Jake, 12, is a cheeky, energetic, and outgoing young chap with a love for swimming, train rides, and water play. He enjoys playing in his sensory room and creating some mischief! Jake lives at home in Bray, Berkshire with his family and has a care team who provide day and night support. He has Autism Spectrum Condition (ASC), a Learning Disability, Development Delay, and a Visual Impairment. When at home, Jake is mobile, and when outside or for longer journeys, he uses his wheelchair. Jake strives for his independence, and his family and team support assist him in reaching all his targets and goals. His team is growing, and he requires a professional and dedicated Care Team Leader to support his team of support workers.
Overview of role: The Care Team Leader will be responsible for managing and running the care team, arranging staff inductions, training, meetings, appraisals, and supervisions. The typical duties include rota management, completing care logs, updating care plans, ordering stock, managing therapy / health care appointments, and liaising with other healthcare professionals, working in collaboration with Jake's family and case management team. The opportunity is available to also work with Jake on a 1:1 basis Monday to Friday. He requires 1:1 assistance and support with his afterschool and bedtime routine, including assistance with personal care, self-help skills, supporting afterschool activities, preparing meals and snacks, assisting and implementing therapy programmes (training provided by Jake's therapist), and arranging fun and engaging learning and play opportunities. During school holidays, you will plan daily outings and trips that Jake can enjoy and participate in. The role will involve supporting Jake with self-regulation and helping him to manage his expressive behaviours.
