Job Description
As a Social Worker in the Adult Care and Hospital Discharge Team, you will play a vital role in supporting adults with a range of needs as they transition from hospital to home or other care settings. Your responsibilities include conducting comprehensive care assessments to evaluate individuals' needs, risks, and strengths, ensuring compliance with the Care Act 2014. You will work closely with patients, their families, and multidisciplinary teams to create personalised care plans that facilitate safe and timely hospital discharges, promoting recovery and independence.
Responsibilities include:
* Conduct comprehensive care assessments under the Care Act 2014 to evaluate individuals' physical, mental, and social needs, particularly during the hospital discharge process, ensuring appropriate post-hospital care.
* Assess mental capacity in accordance with the Mental Capacity Act 2005 for individuals unable to make decisions about their care, and initiate applications for Deprivation of Liberty Safeguards (DoLS) where required to ensure compliance and protect individual rights.
* Coordinate hospital discharge plans, ensuring that individuals have appropriate care arrangements, such as home-based services or residential care, and prevent unnecessary delays in discharge.
* Respond to urgent cases, managing complex discharges in emergencies or crises, ensuring individuals receive timely and appropriate interventions to support their recovery and avoid readmission.
* Monitor and review care plans post-discharge, ensuring that services provided continue to meet individuals' evolving needs and adjust support accordingly.
* Safeguard vulnerable adults, identifying risks of abuse or neglect and taking appropriate safeguarding actions to protect individuals' well-being and safety during the discharge process.
Requirements:
* Relevant social work qualification
* 2 experience in adult social work, including assessment and care planning.
