Job summary Gillingham Medical Practice is looking for a Social Prescriber to work 25 hours a week with children and young people aged 11 to 25 years - supporting them with what matters to them in a non-medical context. Working as part of friendly Wellbeing Team and alongside approachable colleagues at Gillingham Medical Practice. The successful applicant will have experience of working with children and young people and an understanding of social prescribing. This is a unique role at the surgery and a lot of good work has already been done; however, we are looking to develop the role further. The role will include working with others to introduce proactive initiatives to support children and young people build emotional resilience skills for life. They connect people to community groups and agencies for practical and emotional support and offer a holistic approach to health and wellbeing, hence the name social prescribing. Work with The Wellbeing Team to manage and prioritise caseloads, in accordance with the need and prioritise any urgent support required. Build relationships with key staff in the GP practice, attending relevant meetings, becoming part of the team, giving information and feedback on social prescribing. Create and maintain referral routes into the scheme via a wide range of frontline professionals in the health, education, youth justice, social care and community development sectors.
