Job summary We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our busy practice on a locum basis up to 30 hours per week. We can be flexible on working hours. We are seeking an immediate start. The successful candidate MUST have experience of working in a GP surgery that has used SystmOne, as well as being GPHC registered and appropriately insured. We're after a Clinical Pharmacist who is comfortable and confident in performing Level 2 medication reviews, with the aim of reviewing around 8 patients per hour. If there is interest from the candidate, we can look at expanding the job responsibilities once surgery processes have been learnt. Main duties of the job Responsibilities: Undertake Level 2 clinical medication reviews and run poly-pharmacy clinics with patients with multi-morbidities, particularly in high risk groups (eg the frail elderly, renal impairment, hepatic impairment, multiple co-morbidities) Management of medicines of patients discharged from hospital, reconciling and identifying unexplained changes and managing these without referral to a GP. Liaise with colleagues in community pharmacy to align support for medicines adherence such as MUR and NMS Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines Undertake any other duties related to medicines and patient care as decided by the GP Practice.
