Cover the Tower Hamlets Community Diabetes Service, based at Mile End Hospital.
You will be joining an enthusiastic team of Community Diabetes Nurse Specialists and other healthcare professionals, providing clinical and educational support to patients with both Type 1 and Type 2 diabetes, as well as their carers.
The successful candidate will need strong time management, organisational, and communication skills and should be capable of managing a clinical caseload and working independently. We are committed to support personal development and offer education.
At Bart’s Health Community Diabetes Service, we deliver structured Type 1 education through the Dose Adjustment for Normal Eating (DAFNE) programme and Type 2 education through the X-Pert educational programme. Additionally, we provide structured education for those at risk of developing diabetes (Pre-Diabetes Education).
Bart’s Health is a leading teaching centre, welcoming medical students from Queen Mary University of London and nursing students from City and Greenwich University.
The post is an exciting opportunity to work within the speciality of diabetes,contributing primarily to the delivery of the adult diabetes service at Mile End Diabetes Care Centre and within GP practices in Tower Hamlets.
The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for adults with diabetes and their carers.
The post holder will be a clinical and educational resource for patients and health professionals; and will ensure a smooth transition from primary to secondary care occurs in terms of diabetes care. The post holder will be a good team player always striving to improve the care of people with diabetes.
