As a Community Mental Health Practitioner, you will be responsible for: Holding a caseload of patients with organic and functional presentations under CPA and NCPA Having sound knowledge of the Mental Health Act, Mental Capacity Act, NICE guidelines etc. Taking new referrals from GPs, Wards and other mental health teams/services. Working with our primary care partners. Performing initial assessments. and reviews. Assessing and managing risk. Using Dialog+ where possible and Recovery Care Planning with service users and carers. Managing your own diary and planning your day. Attending MDT meetings and ward review meetings. Being part of the duty system, usually once per month. Communicate effectively through clinical notes, documents on Lorenzo and letters to GPs and other involved parties. Administering depot medication. Driving is essential for this role.
