Job summary
In Dudley we believe that for many of our patients, particularly the elderly and frail, Hospital is not the best place for them to be. We are looking to recruit additional Clinical Hub Medics who share our beliefs and who can support our Clinical Hub team to achieve this.
Over the last 3 years the Dudley system has built a team of experienced nurses to help look after housebound patients with long term conditions, a community single point of access, a triage and visiting service for care homes, and a care home education team. This has involved strong partnership working with the CCG, GPs, Black Country Healthcare our mental health provider and Dudley Integrated Health & Care. The post-holder will provide medical support to the Clinical Lead and other community teams within the Dudley Clinical Hub.
The role will include dealing with a wide range of health needs in a community care setting, ensuring the highest standards of care for older adults registered with Dudley CCG practices.
This is an exciting opportunity for either a GP or senior medic with a passion for the care of older people and supporting services in the community to prevent inappropriate admissions to hospital through their work within the Clinical Hub.
Main duties of the job
1. Work closely with their medical, nursing, AHP and management colleagues, colleagues in primary care, neighbouring secondary and tertiary care providers, in the pursuance of the operational and strategic objectives of the Clinical Hub.
2. Providing daily clinical support to the teams in the Clinical Hub to ensure delivery of a safe, clinically effective, efficient and high quality clinical service.
3. To provide support for triage (via both face to face and non-face to face contact) and assessing complex patients including end of life patients to prevent admissions to hospital.
4. Providing training and support to the Advance Nurse Practitioners and other specialist nurses within the Clinical Hub.
5. To be a conduit with primary and secondary care and supporting system wide resilience.
6. To support the development of the Clinical Hub and the development of clinical pathways.

Person Specification
Registration
Essential
* Full GMC registration with a license to practice.
* On the Specialist Register or has CCT within 6 months of interview.
Qualifications
Essential
* Degree from a recognised University.
* MRCP (UK) / MRCGP or recognised equivalent qualification.
Knowledge and experience
Essential
* Ability to take full and independent responsibility for the care of patients and offer expert clinical opinion in the specialty.
* Clinical training and experience equivalent to that required for gaining UK CCST/CST.
