The LTHT Operations Centre is seeking an experienced and motivated senior nurse to play a pivotal role in the leadership and co-ordination of the transfer and discharge of people to an appropriate place of care.
This exciting new post will work with partners across the Leeds Health and social care community, to facilitate timely discharge from hospital with a focus on Home First.
The successful candidate will develop and lead a training and education programme for the multiprofessional/multiorganizational staff groups working in the Hospital to support collaboration and consistency. The post holder will lead and participate in both internal and external meetings, representing LTHT and Leeds Community Health.
The post holder will be an expert practitioner so experience within the field is essential. The successful candidate will need to demonstrate a high level of communication skills and evidence of influencing and leading change across organisational and professional boundaries. Peoples experience of discharge is central to any future development; the post holder will develop strategies to act on feedback to improve the service.

Main duties of the job
1. Facilitating and Co-ordinating the transfer of care and discharge arrangements for patients at LTHT by the use of quality improvement methodology and the Leeds Improvement Method.
2. Development of Nursing Policies, Clinical Guidelines which support transfer of care and discharge and the monitoring of these policies or guidelines through audit.
3. Development of Nursing, Midwifery and AHP roles specific to transfer of care and discharge.
4. To provide monitoring, assurance and on-going development of the LTHT processes for patients with a long length of stay. To work with system partners to ensure developments align with multi-organisation objectives.
5. Work closely with LTHT care delivery partners in the setting up, monitoring and evaluation of key patient quality indicators.
6. Responsible for a team of nurses and AHPs that will work in support of wards and clinical areas in the facilitation of patients transfer of care and discharge from LTHT.

Job responsibilities
1. Lead in the on-going development and monitoring of the process for the transfer of care and discharge for patients within LTHT.
2. Lead in supporting clinical audit and benchmarking, using the Essence of Care toolkit, ensuring examples of good practice is shared.
3. Work in supporting Matrons and Discharge Co-ordinators to address issues in clinical areas.
4. To act as expert Communicator disseminating and receiving feedback on various projects from a wide range of staff (Nursing, Midwifery and Allied Health Professionals).
5. Actively seek and identify opportunities for new ways of working to improve safety, efficiency, and high standards of care.
6. Aim to ensure nursing services are benchmarked against high performing organisations nationally and internationally.
7. Ensure clinical practice follows a sound evidence base.
8. Work with the Head of Nursing for the Operations Centre to ensure the implementation & delivery of the Nursing Strategy via agreed action plans.
9. Act in accordance with the Nursing & Midwifery/Health Care Professions Code.
10. Fulfils the requirements as directed by the Nursing and Midwifery Council or Health Care Professional Council to maintain and revalidate registration.
11. Support the Deputy Chief Nurse and Head of the Operations Centre in the delivery of an enhanced, robust system for monitoring and reporting key standards in relation to transfers of care and discharge.
12. Work with Clinical Service Units and the Nursing Directorate in the delivery and monitoring of key performance indicators based on long length of stay.
13. Lead and promote change in nursing and clinical practice in order to improve staff and patient experience via projects delegated by the Deputy Chief Nurses.
14. Provide a positive role model to nursing staff and ensure involvement and participation of staff groups such as Nursing & Midwifery Operational group.
15. Work with the Nursing Directorate team to promote, develop and deliver a nursing & midwifery leadership strategy across the Trust.
16. Lead on resolving issues with transfer of care and discharge by working with discharge facilitators, ward leaders and staff.
17. Develop effective strategies to support ward leaders and staff in improving practice in the transfer of care and discharge of patients within LTHT.
18. In conjunction with the Deputy Chief Nurse and Head of Nursing for Operations centre and System leaders, support and develop the education agenda in nursing, midwifery and AHPs to promote and assure the delivery of high standards of care in the transfer of care/discharge. To include all professions within the Leeds Place.
19. Lead the team of Discharge Facilitators to deliver support and supervision to clinical areas in the pursuit of the highest standards of care in relation to the transfer of care and discharge.
20. Provide reports to the Deputy Chief Nurse and Head of Nursing for the Operations Centre on the progress and implementation of any projects related to the transfer of care and discharge, including written and verbal reports.
21. In conjunction with CSU Matrons, provide line management for the members of the discharge team and support team.
22. Provide Line Management for the Band 7 Senior Nurse for the LTHT Discharge facilities.
23. Monitor and maintain budgets associated with the role.

* Significant experience at senior level in nursing, midwifery or AHP (Band 7 or above).
* Experience of managing staff Project Management.
* Service Development.
* IT skills, including database and spreadsheet utilisation.
* Understanding of the underlying social, political land historical factors shaping local and national realities of health services.
* An understanding of the organisational structure and working practices of the organisation.
* Understanding of the local and national nursing, midwifery, AHP agenda.
* Awareness of Leeds Teaching Hospitals Trust guidelines and policies.
* Understanding of professional standards and safety standards.
* Experience managing and leading staff.
* Budgetary management experience.
