Alrewas Surgery is looking for 2 experienced Care Navigators Receptionists to join our fast-growing team. The post is for 15 hours per week per person. A long-standing, well-respected GP practice covering a semi-rural area between Lichfield and Burton on Trent, with easy vehicle access as we are situated just off the A38 close to the National Memorial Arboretum. Alrewas Surgery aims to offer a welcoming, high-quality GP service to all its patients. To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team.
The Care Navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.
Main duties of the job
The following are the core responsibilities of the Care Navigator:
1. Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition.
2. Answer incoming phone calls, transferring calls or dealing with the callers' requests appropriately.
3. Process patient requests for appointments.
4. Process repeat prescription requests.
5. Initiate contact with and respond to requests from patients, team members, and external agencies.
6. Photocopy documentation as required.
7. Data entry of new and temporary registrations and relevant patient information as required.
8. Input data into the patients' healthcare records as necessary.
9. Direct requests for information, e.g., SAR, insurance/solicitors letters and DVLA forms, to the administrative team.
10. Manage all queries as necessary in an efficient manner.
11. Carry out system searches as requested.
12. Maintain a clean, tidy, effective working area at all times.
13. Monitor and maintain the reception area and noticeboards.
14. Support all clinical staff with general tasks as requested.
15. Undertake all mandatory training and induction programmes.
16. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

Alrewas Surgery is a semi-rural practice serving 7500 patients. We are a staff team of 32 including GPs, nurses, HCAs, clinical pharmacists, and administrative and management staff.
Job responsibilities
The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
1. Contribute to public health campaigns e.g., flu clinics through advice or direct care.
2. Participate in practice audits as directed by the audit lead.
3. Action incoming emails when necessary.
4. Scan patient-related documentation and attach scanned documents to patients' healthcare records.
5. Complete opening and closing procedures in accordance with the duty rota.
6. As required, support in the process of repeat prescriptions, ensuring that they are processed accurately and efficiently.
7. Order and monitor stationery supplies.
Person Specification
Qualifications
* Educated to GCSE Mathematics & English C or above or equivalent.
* Active signposting or Care Navigator training qualification.
* Healthcare qualification level 2 or working towards gaining equivalent level.
* Experience of working in a primary care environment.
