Position
An exciting opportunity for a part-time Physiotherapist has arisen to become part of the Enhanced Intermediate Care and Rehabilitation Service in South Ayrshire, which is a jointly funded inter-disciplinary team working in partnership with local authority, NHS Ayrshire and Arran
What you will do:
Private and Voluntary Agencies to provide short term support for people with conditions that can be safely managed at home or in community but require additional health and social support to facilitate this.
The service can facilitate early and supported discharge from hospital or provide an alternative to hospital admission.
It aims to maximise the long term independence, choice and quality of life and as a consequence reduce the need for on-going support from formal services.
People’s needs are identified using documentation based on a comprehensive multi-factorial care of the elderly assessment. Interdisciplinary treatments, rehabilitation and enablement plans are then created in partnership with the service user.
The focus is on setting goals and achieving outcomes for individuals that enable them to be as independent as possible

Knowledge, training and/or experience required to do the job:
A Diploma/Degree in Physiotherapy, with experience working within a multi-disciplinary Community Team, good written/ verbal communication skills and an excellent team worker are essential. Evidence of continuing professional development, including extensive experience in relevant clinical field in health and/social care.
You will be required to have basic information technology skills.
