We are excited to share an opportunity to join our team of elderly care consultants here at East Cheshire NHS Trust.
A unique opportunity has arisen for a Locum Consultant Geriatrician to support our developing Care of the Elderly service as well as supporting acute frailty services in line with the Trust's vision.
The Trust is committed to providing coordinated, joined up care for older adults living with frailty across the community-hospital interface. The Trust has recently re-aligned its Crisis Response services with clear opportunities to strengthen links across the interface between the two services. Virtual community ward models are also in early development with the potential to grow these services further with appointment to this role. There is an emphasis on an integrated, whole system approach to older people’s care in East Cheshire as part of the NHS long term plan.
Virtual wards allow patients to receive care at home safely and conveniently, rather than being in hospital. NHS England has an ambition for Frailty Virtual Wards to be rolled out to provide care closer to home to patients who would otherwise be admitted into hospital. We aim to build a virtual ward consisting of 30 virtual beds (in patient’s own homes / care homes). This is an exciting new service that offers successful candidates a unique opportunity to be involved in a new model of care for older people with frailty in East Cheshire.
