Job summary Leamington PCN seek an enthusiastic and proactive Care Coordinator to join their team. Leamington is a patient focused forward thinking PCN looking for people to work with the PCN team to deliver care to their patient populations. The Care Coordinator role is a new and evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work, the role will consist of some focussed project work as well as general core support to the PCN. The posts holder would ideally have good knowledge of community or primary care sector and strong IT skills as the role will be key to practice searches and audits. Main duties of the job Care Coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will support the Clinical directors in coordinating all key activity including improving access to services, providing advice and information and ensuring health and care planning is timely, efficient and patient centered. They will work closely and in partnership with the Social Prescribing Link Workers and support delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management.
