We are looking to recruit an experienced specialist pharmacist to take on a senior role within our Buckinghamshire Lipid Optimisation Programme.
This specialist programme delivers a population-health approach to lipid management across Buckinghamshire. Using specialist population health tools, we have delivered appointments to over 1,500 patients within the first 12 months and offered medication changes to over 1,200 patients. This includes over 500 patients being commenced on injectable lipid lowering therapies within secondary care.
This role will support delivery of the programme, along with providing expert advice, support and training to colleagues within primary care.
They will lead on providing lipid optimisation education to the 47 primary care teams across Buckinghamshire, along with supporting transfer of care to those practices who are keen to deliver injectable therapies closer to the patient’s home. They will be expected to support the wider programme, as we expand to review all secondary prevention patients within Buckinghamshire (with an LDL >C above 2.6).
We are looking for a candidate whose knowledge and experience will ensure they can rapidly achieve our goals of reducing the incidence of cardiovascular disease across Buckinghamshire. The candidate should have previous experience of working on similar cardiovascular/lipid optimisation projects at a senior level, including demonstrating collaborative working with partner organisations (ICS-level, AHSN or similar organisations).
Our successful candidate will be tasked with sharing learning with primary care colleagues across Buckinghamshire. They will lead on providing lipid optimisation education to the 47 primary care teams across Buckinghamshire, along with supporting transfer of care to those practices who are keen to deliver injectable therapies closer to the patient’s home.
